New York's Essential Plan is one of the most generous low-cost health coverage programs in the country. If you earn under a certain threshold and you're between 19 and 64 years old, you can get comprehensive health coverage with no monthly premium, no deductible, and copays as low as $0. As of 2026, that income ceiling is roughly $34,000 for a single person (though an important change takes effect July 1, 2026, read on).

Quick Answer: In 2026, single New Yorkers earning up to $31,300 qualify for the Essential Plan with $0 premium coverage. A family of four can earn up to $64,300. The program covers doctor visits, hospital care, prescriptions, dental, and vision. Enroll any time at NY State of Health (nystateofhealth.ny.gov). There is no annual open enrollment window.

The Essential Plan is New York's Basic Health Program (BHP), a federal option that states can use to offer affordable coverage to residents who earn too much for Medicaid but not enough to comfortably afford marketplace plans. New York was one of the first states to launch it, and enrollment has grown to over 1.5 million New Yorkers. It is administered through NY State of Health, the state's official health insurance marketplace.

What Changed in 2026

This is critical context if you or someone you know is enrolled. The Essential Plan previously covered people with incomes up to 250% of the Federal Poverty Level (FPL). That upper limit for a single person was around $39,125 per year.

Starting July 1, 2026, New York reduced eligibility back to 200% FPL. That means:

  • People earning between 200% and 250% FPL (roughly $31,301 to $39,125 for one person) lost Essential Plan eligibility on July 1, 2026.
  • About 450,000 New Yorkers were affected by this change.
  • Those people may qualify for subsidized Qualified Health Plans through NY State of Health instead. They had until August 30, 2026 to enroll in new coverage.

If your income is at or below 200% FPL, your Essential Plan coverage is not affected. The 1.3 million-plus enrollees below that threshold keep their coverage.

You may qualify for free health insurance.

Essential Plan Tiers: Which One Do You Get?

The Essential Plan is split into tiers based on your income as a percentage of the Federal Poverty Level. Your tier determines your out-of-pocket costs, though all tiers have $0 monthly premiums and $0 deductibles.

Plan TierIncome Range (% FPL)Approximate Annual Income (1 person)Doctor Visit CopaySpecialist CopayGeneric Rx Copay
Essential Plan 4Below 100% FPLUnder $15,060$0$0$0
Essential Plan 3100% to 138% FPL$15,060 to $20,783$0$0$0
Essential Plan 2138% to 150% FPL$20,783 to $22,590$0$0$1 to $3
Essential Plan 1150% to 200% FPL$22,590 to $31,300$15$25$6 to $30

All tiers include dental and vision benefits at no additional charge, which is uncommon for marketplace-level coverage. Hospital admission carries a $150 copay under Essential Plan 1, while Plans 2, 3, and 4 have $0 hospital copays.

Who Qualifies for the Essential Plan

To be eligible as of 2026, you must meet all of the following:

  • Be a New York State resident
  • Be between 19 and 64 years old (children under 19 may qualify for Child Health Plus instead)
  • Be lawfully present in the United States
  • Not be eligible for Medicaid (if you qualify for Medicaid, you'll be enrolled there first)
  • Not have access to qualifying employer-sponsored coverage
  • Not be enrolled in Medicare
  • Meet the income limits listed above (at or below 200% FPL after July 1, 2026)

You do not need to be a U.S. citizen. Lawfully present immigrants, including DACA recipients in some cases, may qualify. Undocumented immigrants do not qualify for the Essential Plan, but may have other options through emergency Medicaid or local programs.

What the Essential Plan Covers

The Essential Plan covers the same 10 essential health benefits required of all ACA-compliant plans:

  • Preventive care (free, no copay on any tier)
  • Doctor and specialist visits
  • Inpatient and outpatient hospital care
  • Emergency services
  • Mental health and substance use disorder treatment
  • Prescription drugs
  • Lab tests and imaging
  • Maternity and newborn care
  • Rehabilitative services
  • Pediatric services including dental and vision for children

What makes the Essential Plan stand out is the inclusion of adult dental and vision coverage, which standard ACA marketplace plans typically do not cover. Dental benefits include preventive care and basic services. Vision includes annual eye exams and an allowance toward glasses or contacts.

There is no annual deductible on any Essential Plan tier. That means your coverage starts working on day one without having to spend hundreds or thousands of dollars first.

How to Apply in 2026: Step-by-Step

The Essential Plan has continuous open enrollment. You can apply any time during the year. There is no deadline to meet, and no need to wait for open enrollment season.

Step 1: Gather your documents

Before you start, have the following ready for each person applying:

  • Social Security number (or immigration document number if applicable)
  • Date of birth for all household members
  • Proof of income (pay stubs, tax returns, or a written statement if self-employed)
  • Current health insurance information if you have any

Step 2: Apply through NY State of Health

You have three options:

  • Online: Go to nystateofhealth.ny.gov and create an account. The application walks you through each step and automatically determines which program you qualify for.
  • By phone: Call 1-855-355-5777 (TTY: 1-800-662-1220). Representatives are available Monday through Friday, 8 a.m. to 8 p.m., and Saturday, 9 a.m. to 1 p.m.
  • With in-person help: Free certified enrollment assistors are available across the state. Community Health Advocates (1-888-614-5400) can connect you with local help.

Step 3: Submit your application

The system will verify your identity and income. For most people, income verification is done automatically through IRS data. You may need to submit documents if your income changed recently or if electronic verification cannot confirm your information.

Step 4: Choose a plan

Once determined eligible, you'll choose from participating Essential Plan insurers in your area. In New York City, options typically include MetroPlus, Healthfirst, EmblemHealth, and UnitedHealthcare Community Plan. Upstate options vary by county. All plans within the same tier have identical cost-sharing. The main differences are network and available doctors.

Step 5: Coverage starts

If you enroll between the 1st and 15th of the month, coverage typically starts the first day of the following month. If you enroll between the 16th and the end of the month, coverage starts the first day of the month after that. So enroll as early in the month as possible.

The 2026 Transition: If You're in the 200-250% FPL Range

If you previously had Essential Plan coverage with income between 200% and 250% FPL, your coverage ended July 1, 2026. Here is what you need to know:

You should have received a notice from the state around April 2026 informing you of this change and outlining your options. If you missed that notice, contact NY State of Health directly at 1-855-355-5777.

Your main option is a Qualified Health Plan (QHP) through NY State of Health. Because your income falls between 200% and 250% FPL, you qualify for premium tax credits and cost-sharing reductions. In practice, this means your monthly premium may still be affordable, potentially under $50 to $100 per month depending on household size and the plan you choose.

The key difference from the Essential Plan: QHPs have deductibles. A Silver plan with cost-sharing reductions will have lower deductibles than a standard Silver plan, but you will have some out-of-pocket exposure before coverage kicks in. That is the trade-off.

You have until August 30, 2026, to enroll in a new plan and avoid a coverage gap.

Essential Plan vs. Medicaid vs. ACA Marketplace

New Yorkers often get confused about the three main low-income coverage options. Here is how they compare:

FeatureMedicaidEssential PlanACA Marketplace (Silver)
Monthly premium$0$0Varies (subsidized)
Deductible$0$0$100 to $2,000+
Income limit (single)Up to 138% FPL ($20,783)Up to 200% FPL ($31,300)No upper limit
Who it servesLowest incomeLow to moderate incomeModerate to higher income
Dental/visionCoveredCoveredUsually not covered
Open enrollmentYear-roundYear-roundNov 1 to Jan 15 (SEPs available)

If your income is at or below 138% FPL, apply for Medicaid first. It offers the most comprehensive coverage at no cost. If you earn between 138% and 200% FPL, the Essential Plan is usually your best option before considering marketplace plans. For reference, New York's ACA income limits are detailed at CoveredUSA's ACA income limits page.

Frequently Asked Questions

Can I get the Essential Plan if I'm undocumented?

No. The Essential Plan requires lawful presence in the United States. Undocumented immigrants do not qualify. However, New York has other programs. Emergency Medicaid covers emergency care. Some counties and city programs offer additional options. Call 311 in New York City for a referral to local resources.

Does the Essential Plan cover prescriptions?

Yes. All Essential Plan tiers cover prescription drugs. The tier you're on determines your copay. Essential Plan 4 and 3 have $0 copays for all drugs. Essential Plan 2 charges $1 for generics and $3 for preferred brand-name drugs. Essential Plan 1 charges $6 for generics, $15 for preferred brands, and $30 for non-preferred brands.

Can I keep my current doctor?

It depends on your doctor's network. When you choose an Essential Plan insurer, you'll select from that insurer's provider network. If your doctor is in that network, yes. Before finalizing your plan choice, use the insurer's online directory to confirm your doctor participates. You can switch plans during your annual renewal period if your doctor leaves the network.

What if my income changes during the year?

Report the change to NY State of Health as soon as it happens. If your income drops below 138% FPL, you may be moved to Medicaid. If it rises above 200% FPL (after July 1, 2026), you will lose Essential Plan eligibility and need to transition to a marketplace plan. Reporting changes promptly prevents overpayment issues and coverage gaps.

Is there a waiting period for pre-existing conditions?

No. The Essential Plan cannot deny coverage or charge more based on pre-existing conditions. Coverage for existing conditions starts the day your plan takes effect.

How is the Essential Plan different from Obamacare?

The Essential Plan is actually separate from the ACA marketplace (Obamacare). It is a Basic Health Program funded by federal and state dollars, offered only by states that opt in. New York chose to offer it. The Essential Plan provides more generous cost-sharing than typical marketplace plans and has year-round enrollment, while the ACA marketplace has annual open enrollment windows and typically charges premiums even with subsidies.

What if I already have employer insurance?

If your employer offers qualifying health coverage (generally defined as coverage that costs less than 9.02% of household income in 2026 and meets minimum value standards), you are not eligible for the Essential Plan. If your employer's plan is unaffordable by federal standards or does not meet minimum value, you may qualify.
